10mins Online Survey on Health-Related Behavior Change from University of Washington

We would like to invite you to participate in a University of Washington research study on health-related behavior change. In this study, we aim to identify the essential elements that motivate the positive behavior change in unhealthy behavior and determine specific design requirements for technology to help with this process.

If you have undergone significant health-related behavior change, such as losing weight or quitting smoking, and are 18 years old or older, please share your experiences in our approximately 10 to 15 minute online survey by clicking https://catalyst.uw.edu/webq/survey/ycsung/172232. Your experience would be extremely valuable and would assist in the development of technology to facilitate positive behavior change in other people.

By completing this survey you will have the opportunity to win a $50 Amazon.com gift card. Participation in the study is completely voluntary. All information about the study and study participation will be kept confidential.

Thank you for your participation.

Sincerely,
