May be my hobby app for self surveys could be helpful. (Available in free and pro versions). User can define surveys as sets of questions and answers, import/export questions and results in csv, chart the data on android. Any feedback is welcome.

Also, why would someone create questions like “Blood Pressure Sys morning” rather than “Blood Pressure Sys”, or “Cups of Coffee per Day” rather than “Cup of Coffee”, given that each record is timestamped?
Finally, I assume you could record the actual blood pressure values, rather than “2”, “3”, “5” etc?

My idea it the questions belong to the set which is run by the user daily. It is important to compare (at least in my case) blood pressure taken at the more or less same time on the different days, but it makes not so much sense to compare BP in the morning and in the evening.

This is done for two reasons. First, there is no value in differentiating BP e.g. 120 from 122, such difference with the normal “home” way of measuring is just “noise”. Second, it is much faster to tap in a box then to enter three digits. But you are right, a user can make a survey question which will have a box for individual value. This is fully customiziable by the user, he can enter any questions and any answers in the survey template. I just don’t need it, therefore my sample survey looks like this. If you mean that instead of “1”, “2” I could write “120-125”, “125-130” - the values behind the selected ranks - yes, this is possible, just makes a bit more difficult working with the graphs.
